Transitional care program

The transitional care program (also known as the swing bed program) at West Valley Hospital supports patients recovering from serious illnesses or surgery. When you no longer need intensive medical attention, but you aren't yet ready to go home, transitional care can help you build your strength and practice the daily living skills that will set you up for success.

West Valley Hospital works closely with whichever hospital you are coming from to ensure a safe transition into the program.
