West Valley Hospital’s transitional care program (also known as the swing bed program) offers hospital-level skilled care for patients who need extra time to recover before returning home. Whether you are recovering from a serious illness, injury or surgery, transitional care will help you build your strength, improve mobility and practice the daily living skills that will set you up for success.

Planning for your safe return home begins early and continues throughout your stay.

 

Skilled nursing care


Our nurses provide care based on your providers’ orders and your personal needs. This may include:

  • Wound care
  • IV therapy
  • Monitoring your condition
  • Teaching you and your family how to manage care at home


Care management and care planning


Our team works closely with you and your caregivers to:

  • Review admissions and coordinate your stay.
  • Create a clear care and discharge plan.
  • Schedule follow-up appointments.
  • Connect you with community resources or financial assistance, if needed.

Rehabilitation and therapy services

Our post-acute rehabilitation program delivers coordinated physical, occupational and speech therapy designed to meet your unique recovery goals. Our team collaborates with experts from all areas of the hospital to create personalized treatment plans that reflect your abilities, tolerance and medical needs.

We gradually adjust the intensity of your therapy to promote safe, measurable, functional improvement. Our focus is on helping you get back your mobility, independence in daily activities and communication skills — returning you to the highest level of function possible.

 

A team approach to care

Your care is guided by an interdisciplinary team that may include medical providers, nurses, care managers, therapists, and nutrition and pharmacy staff. This team meets regularly to review your progress and plan next steps.

Family involvement

We welcome family involvement and encourage loved ones to be part of your care when helpful. Families may also attend care meetings to stay informed and support recovery.

 
 

Outcome measures

We track our patients' outcomes using standardized measures, including functional gains, goal achievement, discharge to the community, length of stay and patient satisfaction — ensuring high-quality care and meaningful results.

2025 data:

  • Average length of stay: 14.44 days (national average is almost double)
  • Percentage of patients that return to a home setting in the community: 75.22% (national average is only 51%)
  • Percentage of readmissions back to the hospital within 30 days: 19.92%
