West Valley Hospital’s transitional care program (also known as the swing bed program) offers hospital-level skilled care for patients who need extra time to recover before returning home. Whether you are recovering from a serious illness, injury or surgery, transitional care will help you build your strength, improve mobility and practice the daily living skills that will set you up for success.

Your care is guided by an interdisciplinary team that may include medical providers, nurses, care managers, therapists, and nutrition and pharmacy staff. This team meets regularly to review your progress and plan next steps.
We welcome family involvement and encourage loved ones to be part of your care when helpful. Families may also attend care meetings to stay informed and support recovery.
We track our patients' outcomes using standardized measures, including functional gains, goal achievement, discharge to the community, length of stay and patient satisfaction — ensuring high-quality care and meaningful results.

Patients who have received inpatient care or surgery in another hospital system outside of Salem Health are still welcome to choose West Valley Hospital's swing bed program for their rehabilitation. Talk to your care manager for more information.
