Change to indications for restricted antibiotics

12 Sep 2021

By: Matthew Tanner, PharmD, BCPS

All antibiotics require an indication for use at Salem Health, but some can only be used for certain indications (i.e. daptomycin or meropenem).

To make it more obvious which antibiotics are restricted to only certain indications, the indications field at the top will be made blank, and the approved indications will be shown as buttons in the body of the order (see pictures below). This knowledge can help you prevent a page from pharmacy asking you to choose an alternative antibiotic because the indication chosen isn’t among the approved indications.

Future State (starting Sept. 14):

future state indications for restricted antibiotics

  • Indications can be found near the bottom of the order
  • Presented as buttons

Current state:

current state indications for restricted antibiotics

  • Indications are found at the top of the order
  • It isn’t always clear that this is an antibiotic which is limited to only certain indications

  • Aztreonam
  • Ceftazidime
  • Ciprofloxacin
  • Daptomycin
  • Ertapenem
  • Eravacycline
  • Fidaxomicin
  • Fosfomycin
