Community Health Needs Assessment
Community health improvement at Salem Health
Caring for our community
Salem Hospital and West Valley Hospital have had a long-standing commitment to serve those who live, work and play in Marion and Polk counties. The commitment has grown and evolved through significant thought and care in considering our community's most pressing health needs.
The hospitals work with community partner agencies — Marion County Health Department, Polk County Health Department, Kaiser Permanente, Legacy Health, Santiam Hospital, PacificSource, Willamette Health Council — to complete a comprehensive Community Health Needs Assessment (CHNA or CHA) for each hospital facility. The results of the CHNA inform the Community Health Improvement Plan (CHIP), guiding Salem Health’s community benefit and strategic planning.
Through our CHNAs and CHIPs, Salem Hospital and West Valley Hospital identify the greatest health needs among each of its hospital's communities, enabling Salem Health to allocate resources toward services, outreach, prevention, education and wellness opportunities where the greatest impact can be realized.
