The main lines of the fishbone are: Insertion; infection work-up for a patient with a CL; high risk patients; maintenance/documentation and PICC team. Under insertion there is choice of insertion site, preparation for insertion, physical insertions and are line needed are there other options? other devices - mid-lines verses CL. Under Infection there is is source identification, proper use of fever panel tool, antibiotic timing, lab and phlebotomy processing, awareness of NHSN guidelines and priority to isolate source Under high-risk patients there is oncology best practice; care planning; immunocompromised TPN. Under Maintenance/Documentation there is care of the site; daily maintenance; frequency of changing caps and extension sets; care of injection ports; hand hygiene; RN documents cares and baths with accurate assessment data on site; MD daily notes include justification of need, line days and removal target. Under PICC team there is insertion; maintenance and monitoring, rounding; best practice, alternative devices.
