Your question: Why do I keep getting requests for H&Ps to be dictated on outpatient surgeries when they have already been operated on and have signed H&Ps and preop notes? This has been happening since the upgrade to Epic. All it is doing is adding unnecessary time and work.
Our answer: Thank you for bringing this issue to our attention. We have identified a problem with Pre-Surgery H&P workflows. HIM is actively working with Pre-Surgical Screening to resolve it, to ensure that we are compliant with Joint Commission and Medical Staff standards, as well as to prevent the error from flowing forward to HIM.
