The West Valley Hospital Antimicrobial Stewardship (AMS) Program has been working hard to achieve these goals:

  • Optimize antimicrobial selection, dose and duration of treatment to improve patient outcomes.
  • Minimize adverse drug reactions or unintended negative consequences of antimicrobial use.
  • Reduce morbidity and mortality of our patients.
  • Reduce the emergence of resistance.

A core strategy used by WVH AMS Program is a restricted formulary. The WVH Anti-Infective Formulary Management Guidelines (AFMG) provides guidance on specific agents and their indications for use.

At West Valley Hospital, we have two types of restrictions:

  • Restricted by indication
  • Restricted by indication and provider (West Valley Hospital)

 

 

 

 

The WVH Anti-Infective Formulary Management Guidelines (AFMG) helps limit inappropriate ordering of selected agents by providing guidance on acceptable indications. For antimicrobials that require an approved provider and indication, providers should choose WVH as the approved provider option. However, these are agents that would be orderable by infectious disease providers only at Salem Hospital — WVH does not have infectious diseases consulting services.

The appropriateness of ordering is regularly reviewed by the WVH AMS team and follow up will occur as appropriate.
