Warfarin time changes to 1800 daily

To better match what most of our patients do at home, the daily Warfarin administration time will change to 1800 (historically it’s been given at 1400). The change began on March 20.

Pharmacy is also working to increase the amount of DVT prophylaxis (enoxaparin shots) done at the same time to create an “anticoagulant hour” of sorts – but that’s just in the planning stage.
