As of July 1, the Salem Health Professional Services entity has been consolidated into the larger Salem Health organization.
Providers will now be operating under the Salem Health Tax ID, organizational NPI and business entity name of Salem Health. Going forward, please use the appropriate Salem Health identifiers for patient referrals (TIN: 93-0579722; NPI: 1265431829), and cease the use of the “Salem Health Professional Services” entity name, Tax ID, and organizational NPI (TIN: 75-3175249; NPI: 1285774257).
For the time being, patient-facing communications (statements, letters, etc.) will use “Salem Health Medical Clinic” labeling to reduce confusion for patients regarding billing. We don’t expect Insurance payer claim reimbursement to be affected.
Salem Health Medical Clinics associated with Salem Health West Valley Hospital will not be affected by this change, and will still use the Salem Health West Valley identifiers (TIN: 431960221; NPI: 1245237486).
