To All Credentialed Providers at Salem Health & West Valley Hospital
In an effort to make vaccination status more visible, the white stickers that were given to employees, providers and volunteers when vaccinated with the first dose of COVID-19 will now be replaced by green stickers for those who have been fully vaccinated. Please visit the Medical Staff Office (Building B, third floor) at your earliest convenience for your new green sticker.
If you have not been vaccinated, please email the Medical Staff Office at MedicalStaffOffice@salemhealth.org to confirm: a) that you are declining the vaccine at this time or, b) whether you intend to become vaccinated at a later date.
