Lab testing changes: New ARUP test options and code changes

To better serve patients with faster service or reflex testing, the following tests have transitioned to our reference lab, ARUP. Here are the new test options and lab test codes changes:

SERUM PROTEIN ELECTROPHORESIS (SPEP), IMMUNOFIXATION ELECTROPHORESIS (IFE)

Monoclonal Protein Detection, Quantitation, Characterization – top recommendation
- Includes SPEP, IFE, IgA, IgG, IgM, Quantitative Free Light Chain
- Most sensitive battery for the diagnosis of monoclonal gammopathy

Monoclonal Protein Detection Quantitation and Characterization
- Includes SPEP, IFE, IgA, IgG, IgM

Protein Electrophoresis with Reflex to Immunofixation Electrophoresis Monoclonal Protein Detection, Quantitation & Characterization, IgA, IgG, IgM, serum
- Includes SPEP
- If patterns from serum protein electrophoresis are monoclonal or suspicious, then Immunofixation Electrophoresis will be added. Additional charges apply.
- IFE is a more sensitive method for determining and defining monoclonal proteins. No additional testing will be added for proteins that are normal, polyclonal, or that lack monoclonal patterns.

- Includes SPEP only

URINE PROTEIN ELECTROPHORESIS (UPEP), URINE IMMUNOFIXATION

Bence Jones Protein, Quantitation and Characterization, with Reflex to Kappa/Lambda Free Light Chains with Ratio, Urine
- Includes Semi-Quantitative Electrophoresis/Qualitative Immunofixation Electrophoresis
- If Paraprotein%, Urine is resulted as trace or with a numeric value, then Kappa and Lambda Free Light Chains, Urine will be added. Additional charges apply
- Collect: 24-hour urine. Refrigerate during collection. Also acceptable: Random urine.

CEREBROSPINAL FLUID (CSF), IgG PANEL (MULTIPLE SCLEROSIS PANEL)

Oligoclonal Band Profile

Preferred test in the workup of multiple sclerosis.

Detect unique IgG oligoclonal bands in cerebrospinal fluid (CSF) in conjunction with a matched serum specimen. Calculate CSF IgG index.
Collect CSF AND serum separator tube or plain red. Serum specimen should be drawn within 48 hours of CSF collection.

ANTI NUCLEAR ANTIBODY (ANA) SCREEN

ARUP uses Hep2 IgG which has a lower false positive rate compared to the Hep2 polyclonal substrate used by Salem Health.
