Kidney Care joins the Community Connect family

On Feb. 22, Kidney Care Physicians joins Salem Clinic and Oregon Heart as a Community Connect partner.

Community Connect partnerships are a means for creating collaborative health care networks in the local area. It allows Salem Health Hospital to extend Epic to community health care practices. This integrated health record system improves the health care of the community and our shared patients.

Kidney Care Physicians was founded on the idea that early diagnosis, attentiveness to patients' needs, and dedication to patient education will give a high quality of life to our renal patients. Their mission is to provide a standard of excellence in renal patient care to improve the quality of life for our patients. This mission aligns well with Salem Health’s mission to improve the health and well-being of the people and communities we serve.

How will this affect my work?

• Lab and imaging will now receive orders electronically in Epic (no more faxed orders)! This means they will follow current Salem Health work flows for receiving CPOM orders and referral records.
• Kidney Care will now be available as an option in the electronic order and referral processes. This order is called Referral to Nephrology.
• You will now see Kidney Care’s added patient data (clinical documentation, studies performed exams, etc.) when you view your patients charts.
