Herpes Simplex Virus (HSV) antibody IgM testing stopped

25 Aug 2024

Changes based on recommendations from CDC, IDSA, AAP

By: Tish Gross, laboratory and imaging outreach supervisor

ARUP Laboratories (Salem Health’s reference lab) deactivated herpes simplex virus (HSV) antibody IgM testing to align with testing guidelines on Aug. 19.

The Centers for Disease Control and Prevention (CDC), Infectious Diseases Society of America (IDSA) and American Academy of Pediatrics (AAP) do not recommend IgM antibody testing for the diagnosis of HSV.

This means these test codes are being removed:

  • XHSVM,  ARUP Code: 0050641 Herpes Simplex Virus Type 1 and/or 2 Antibodies, IgM by ELISA
  • XHVGM, ARUP Code: 0050291 Herpes Simplex Virus Type 1 and/or 2 Antibodies, IgG and IgM
  • XHSVP,   ARUP Code: 0050916 Herpes Simplex Virus Type 1 and/or 2 Antibodies, IgG and IgM with Reflex to Type 1 and 2 Glycoprotein G-Specific Ab, IgG

ARUP Laboratories recommends these tests instead:

  • Preferred test for HSV oral, genital, and CNS infections:
    • Epic LAB3904, XHV12, ARUP Code: 2010095 Herpes Simplex Virus (HSV-1/HSV-2) Subtype by PCR
  • Preferred testing for herpes simplex virus (HSV) when exposure to HSV is suspected:
    • Epic LAB4453, XHVGS, ARUP Code: 0051708 Herpes Simplex Virus Type 1 and/or 2 Antibodies, IgG with Reflex to Type 1 and Type 2 Glycoprotein G-Specific Ab, IgG
  • Preferred serology test for diagnosing herpes simplex viral infection, in the absence of active lesions:
    • Epic LAB4469, XHVRG, ARUP Code: 0051152 Herpes Simplex Virus Type 1 and Type 2 Glycoprotein G-Specific Antibodies, IgG by CIA
