You have until Nov. 12 to either consent and receive the flu vaccine or complete your exemption form. Look for the consent form in your MyChart account. For the provider exemption form, visit Salem Health’s NFLH site, while logged into the intranet.

You can get your flu vaccine from a flu champion on your unit, at a Salem Health or West Valley Tailgate, or by making an appointment at Employee Health 503-814-7250.
