Did you use Building A lab’s Chemistry Analyzer Oct. 3 or 4?

Some results affected; retesting available at no charge

By: Andrew Judd, MD, Medical Director, Laboratory Services; Jamie Rouse, Director, Laboratory Services; and Amanda Jacobson, Senior Specialist Chemistry

Salem Health Laboratory – Building A identified an issue with one of their Chemistry Analyzers.  Some patient results were affected. This issue lasted about five hours, from 9:30 p.m. Thursday, Oct. 3, until about 2:30 a.m. Friday, Oct. 4.

Due to the intermittent nature of the issue, it is difficult to determine which samples or tests were affected. Some samples have been repeated and corrected reports issued. However, not all samples tested during this time can be repeated.

The variance occurred at the Salem Health Laboratory – Building A, which primarily serves inpatients. Most outpatients, even if their specimen collection was performed on the Salem Hospital campus, are tested at our State Street lab and are not affected.

Salem Health Laboratories is committed to providing quality results for your patients. Repeat testing is available if you feel one of your patients was affected by this issue.

Repeat testing must be completed on a newly collected sample and will be performed once at no cost to the patient. If you have a patient you would like to be retested, please contact Client Services at 503-814-LABS (5227).

A client service representative will facilitate the retesting of patients. Please do not order repeat testing in Epic or through any other electronic or manual requisition process. No charge testing can only be accessed by calling client services directly. This opportunity will be available until Friday, Nov. 1.
