The Salem Hospital Credentials Committee is seeking nominations for four positions; three members of the active staff (MD/DO/DPM/DMD) and one credentialed and privileged Advanced Practice Clinician (e.g. CNM, NP, PA) at Salem Hospital. 

Our current members, Dr. Maziar Azadpour, Dr. Andrew Cleary, Dr. Michael Pass and Jacob Nicholson, FNP have expressed interest in serving another term.  They understand the importance of sharing the opening of these positions with all members of the medical staff and APCs that may be interested. 

Minimum Qualifications: 

  • Physician Member:  member of the Active staff category as an MD/DO/DPM/DMD at Salem Hospital
  • Advanced Practice Clinician member: credentialed and privileged Advanced Practice Clinician (e.g. CNM, NP, PA) at Salem Hospital
  • Preferably involved in past or current medical staff leadership

Responsibilities: 

  • Review and recommend action on all applications and reapplications for membership and status on the Salem Hospital medicals staff
  • Review and recommend action on all voluntary requests for changes in privileges for practitioners granted privileges at Salem Hospital
  • Recommend criteria for the granting of medical staff membership and clinical privileges for Salem Hospital
  • Develop, recommend and consistently implement policy and procedures for all credentialing activities at Salem Hospital
  • Perform such other functions as requested by the Medical Executive Committee

The Credentials Committee meets two Mondays a month from 6:30 to 8 a.m. Specific days TBD.

All nominations will be reviewed by the MEC and a ballot will go out to the respective Active staff category or Advanced Practice Clinicians.
