Correction dose for insulin now in handy table

The Lab identified a problem with insulin dosage readability and conducted an SBAR for a solution.

Situation: Patients on discharge should be able to correctly identify the exact dose of correction scale (previously sliding scale) insulin, based on their blood sugar levels.

Background: The correction scale prints in the AVS as a text paragraph of medication details.

Assessment: It’s hard to read, leading to difficulty in determining dosage.

Recommendation: A new set of smart text options (example below) is available for correction scale insulin, depending on intensity of treatment (mild, moderate, etc.). Doses are easier to read in the AVS now, which is available to providers who are involved in discharge.
