Anticoagulation reversal pharmacy protocol

There is a new order which will help providers order anticoagulation reversal more easily:  Anticoagulation Reversal (drug and dose) per Pharmacy Protocol. Using this order will help easily order the right med to reverse an anticoagulant while minimizing clarifying phone calls from the pharmacist. The go-live date was Oct. 4.

This order refers to the pharmacy Anticoagulation Reversal Protocol. The medications covered by the protocol will appear as buttons to choose.  If warfarin is the medication to be reversed, you will be asked for the reason to help choose the “correct PCC” for the patient:

  • moderate bleeding or emergent/urgent surgery
  • life-threatening bleeding or hemodynamic instability
