21st Century Cures Act changes: Your notes will be shared

As part of the 21st Century Cures Act, clinician notes will automatically be shared with patients. As of April 7, you can prevent sharing for specific patient harm prevention and privacy reasons. Approved categories and reasons not to share a note include:

  • Preventing Harm: In my professional opinion based on my relationship with this patient, I have determined that information contained in this note may pose a risk to the life or physical safety of this patient.
  • Preventing Harm: In my professional opinion based on my relationship with this patient, I have determined that information contained in this note may pose a physical safety, reputational or emotional harm to a family member or another patient referenced in the EHI.
  • Privacy: This patient is currently in a correctional facility and this care was received during the course of incarceration.
  • Privacy: This patient is part of an ongoing research study.
  • Privacy: The patient requests that the information not be shared.
