Medication Management

Salem Health's Medication Management clinics provide monitoring, medication adjustment and education for people with chronic medical conditions that are often best managed by taking one or more medications.

Teams of highly trained, expert clinical pharmacists manage our clinics. Using our proven process, we’ll be your advocates to ensure your medications are optimized to be the best, and most effective option for you and your lifestyle.

Who benefits from medication management services?

Chronic conditions that we help manage include: 

  • Diabetes
  • High blood pressure
  • High cholesterol
  • COPD/smoking cessation
  • Osteoporosis
  • Hepatitis C
  • Anxiety and depression
  • Polypharmacy (help reducing the amount of medicines you take)

To become a patient of our clinics, you need a referral from a primary care provider (MD, DO, FNP, ANP, NP or PA) from a Salem Health Medical Clinic or Salem Clinic.  

If your provider is not a Salem Health Medical Clinic or Salem Clinic provider, please refer them to the provider section below.

Medicare and most insurance plans cover the cost of visits to the clinic for most patients. We encourage you to contact your insurance provider to find out about your coverage and any copays or coinsurance fees associated.

Important information for patients

Your first visit to the medication management clinic will be 60 minutes in length. Please bring your updated medication prescriptions or a list of medications to this appointment. Moving forward, your appointments will take approximately 30 minutes.

What to expect during your visit:

  • We help you get started as a new patient by providing in-depth education.
  • We review your health history, current medications and diet.
  • Assist you with understanding medications and your individual treatment plan.
  • Provide nutrition and lifestyle recommendations.
  • We discuss changes in diet, acute illnesses and medications that could affect your therapy.
  • For patients with chronic conditions, we prescribe you needed medications and send them to your preferred pharmacy.

When to call the clinic

Please give us a call or message us through MyChart for any of the following concerns:

Medication issues
  • Questions about newly prescribed medication.
  • Medication refills (only medications we write for you). Please call your prescribers for medications not managed by our clinic.
  • Concerns about medication related side effects.
Health changes
  • Newly diagnosed conditions.
  • Any procedures coming up — including dental work.
  • Any illness lasting longer than three days.
Changes with provider
  • When a health care provider tells you to start, stop or change your medications.
  • When you are changing primary care providers.
  • When you are receiving or plan to receive home health services for any reason (including physical therapy, occupational therapy or nursing services).
  • If you were admitted to the hospital or evaluated at an emergency department since your last visit to the clinic.

Are you a primary care provider for Salem Health Medical Clinics or Salem Clinic who would like to refer your patients?

In Epic, use CON1117 “referral to med management clinic” and select one of the pre-specified diagnoses you would like our team of clinical pharmacists to help manage.
