For your convenience, we offer an easy-to-use online registration form. Just click on the link below to get started. You will need all of your insurance information in order to complete the online registration process.
All information we receive will be kept private. The online form has the same privacy and security protection we use for all medical records. We take patient privacy seriously!
A normal online registration can take 15 minutes or less. With the exception of Maternity, this form is best utilized if your appointment is within the next 30 days. However this process is not intended for procedures less than 24 hours away.
If you need to register for a procedure that is less than a day away, or just prefer to register by phone, please call the Admitting staff at Financial Clearance: 503-814-7737. Our office is open from 8 a.m. to 4 p.m.
The documents linked below explain some of your rights and responsibilities as a patient. You can print and sign them now (and bring them to the hospital with you) or wait and sign the copies we provide at the hospital.
