Typical primary care services
  • Health maintenance and preventive care for the whole family
  • Physical exams and immunizations
  • Lifestyle counseling (e.g. weight management, diseases, high blood pressure)
  • Well-woman exams, including pap smears and nonsurgical gynecology
  • Pediatric care
  • Cancer screening, including cervical cancer diagnostic tests/biopsies.
  • Minor surgeries for moles, warts, cysts and skin cancers
  • Liquid nitrogen freezing of warts and other skin lesions
  • Vasectomy without stitches, performed in the clinic
  • Laboratory services
  • Cardiopulmonary testing
First visit

Please arrive 15 minutes before your scheduled appointment time. You can expect to spend 60 to 75 minutes in our office as we learn about your health concerns and goals. Be prepared to review and discuss your medical history, current health status, medications and supplements with your doctor.

Be sure to bring:
  • All bottles and packages for medication and supplements you are taking.
  • Your current insurance card and government-issued photo identification.
  • A list of questions or concerns you would like to discuss. (Download a worksheet where you can write down your questions.)

Appointment times

With the exception of your first visit (when you should arrive 15 minutes early), please arrive 5 to 10 minutes early at each visit to complete any necessary paperwork.

If you arrive after your scheduled appointment time, we will make every effort to see you. However, it may be necessary to reschedule your appointment.

Please notify us at least 24 hours in advance when canceling an appointment.

No-show appointments

Missing your appointment without notifying your clinic can set back your health care goals. It also keeps other patients from being able to use that time.

Tips to avoid no-show appointments:

  • Keep an eye on your text messages. You will get a message asking if you want to confirm your appointment. Please make sure to reply.
  • Make sure you add your appointment to your calendar, set an alarm reminder and arrive 15 minutes early.
  • Cancel or reschedule your appointment at least 24 hours ahead of time by giving your clinic a call. That way we can give your time slot to someone else who is waiting.

If you miss three (3) appointments without contacting us, you may be dismissed from our practice.

Referral
Referrals and diagnostic testing

If you need to see a specialist, your family medicine health care team will make the referral and help coordinate the transfer of necessary medical information.

We will continue to manage your overall care and work with the specialist to make sure your health care needs are being met.

Prescription refills

Medication safety is one of our chief concerns. Our providers carefully review prescription refills and may want to speak to you before authorizing a refill.

Pay-bills
Insurance and payment

We accept most insurance plans — including Medicaid and OHP. Copayments, coinsurance and deductibles are required to be paid at the time of service.

For those who prefer self-pay, we offer a discount when payment is made in full at the time of service. Billing for laboratory, radiology and other testing services will be made through Salem Health or the outside provider of your choice.

Patients who don’t have insurance are welcome at Salem Health Medical Clinics. You will be asked to pay a deposit for the cost of services before your appointment and will be responsible for the total cost of your care.

If you are unable to pay a deposit, you may qualify for assistance. Dial 211 on any phone or visit 211.org to be connected to community resources.

We're here when you need us

You can call us anytime — 24 hours a day, seven days a week. During our regular office hours, members of our clinical support team are available to schedule appointments or help connect you with another member of your health care team.

For urgent issues after our normal business hours, our on-call providers will assist you over the phone.
